Service Coordination
For Home and Community Based Services clients (AC, EW, TBI, CAC, and CADI) the Public Health Nurse provides ongoing assistance with and evaluation of
services.
This includes assessment of needs, arranging services, coordinating services, advocacy, problem solving, and future planning.
