Waivered Services


Alternative Care (AC):

  • Includes:  Homemaker, home health aide, adult day care, respite care, service coordination, chore services, assisted living, personal care, adult foster care, home delivered meals, skilled nursing, special supplies and equipment, and residential care services.
  • Eligibilty:   (1) age 65 and over; (2) financially eligible; and (3) at risk for nursing home placement.
  • Note:  A premium may be required.

Elderly Waiver (EW):

  • Includes: Homemaker, home health aide, home modifications, adult day care, respite care, service coordination, chore services, assisted living personal care, adult foster care, home delivered meals, skilled nursing, special supplies and equipment, and residential care services.
  • Eligibility:  (1) age 65 and over; (2) a current Medical Assistance recipient; and (3) at risk for nursing home placement.
  • Note:  A spend down may be required.

Community Access for Disability Inclusion (CADI):

  •  Includes:  Service coordination, assisted living, supported employment, prevocational services, homemaker, adult day care, independent living skills, family counseling, adaptations, respite care, extended home health care services, home delivered meals, adult foster care, special supplies, equipment, and home modifications, and residential care services.
  • Eligibility:  (1) under age 65; (2) a current Medical Assistance recipient; (3) certified as disabled; and (4) at risk for nursing home placement.

Brain Injury (BI):

  • Includes:  Service coordination, assisted living, supported employment, prevocational services, adult day care, behavioral programming, cognitive rehabilitation therapy, extended home health, personal care, family counseling and training, foster care, homemaker, home modifications, independent living skills and therapies, respite care, residential care, structured day program, special supplies, equipment, and transportation.
  • Eligibility:  (1) under age 65; (2) certified as disabled; (3) diagnosed with a brain injury (not congenital or degenerative) with significant behavioral and cognitive problems realted to the injury; (4) at risk for nursing home placement or requires neurobehavioral level of care; and (5) eligible for Medical Assistance.

Community Alternative Care (CAC):

  • Includes:  Service coordination, respite care, family counseling, extended hours of nursing care and therapy, special supplies and equipment, modifications and adaptations to home, and medical transportation.
  • Eligibility:  (1) under age 65; (2) eligible for Medical Assistance; (3) at risk for long term hospitalization; and (4) certified as disabled.
