Our Purpose:

The purpose of this collaborative, is to support coordinated planning and service delivery efforts to children and their families, through a prevention and early intervention model.

What we do:

• Provide early intervention/prevention to children and their families in a setting most comfortable to them
• Provide information and referral services
• Provide community outreach
• Collaborative services, supports and resources
• Ensure an equal decision making role for parents, caretakers and guardians, in the planning of supports and services for children and families
• Care plans are based on each child an family's strengths, culture, values and preferences
