Services for Individuals with Developmental Disabilities/ Related Conditions:
Le Sueur County Human Services provides support and assistance to individuals with disabilities including, but not limited to: Intellectual Disabilities, Physical Disabilities, Autism, or other related conditions.
To assess potential eligibility, you will meet with a social worker who will assist you through the process. After a determination of eligibility is made, social workers work with individuals to access needed services and supports such as: case management, and home and community based services. Our goal is to assist individuals to live as independently as possible in their home or community setting.
Who is eligible:
Eligibility is determined through a Rule 185 evaluation which includes school, medical, and psychological records, full scale IQ scores and adaptive behavior findings. Information based on these documents would determine eligibility. A social worker will assist you through this process.
MnCHOICES Assessments:
Le Sueur County Human Services provides MnCHOICES assessments. A MnCHOICES assessment determines eligibility for long-term care services and supports.
