Le Sueur County Public Health is Medicare certified and licensed by the State of Minnesota to provide home health care services.
About Home Health Care
Home Health Care is professional, caring services at home. Home care teams provide a full range of health care services to the patient and family in familiar and comforting home surroundings. They not only provide care, they teach families what they need to know to help them care for their family member.
Home care promotes family integrity and the independence of the individual in need of care. Home care is best suited for individuals who do not need the 24 hour, around the clock supervision and monitoring that only hospitals and nursing homes can provide. The program of care can be short or long term, and it’s helpful for all ages.
Referrals
Referrals are often made by the physician or hospital but patients and families can seek these services on their own as well.
Skilled Nursing
Registered nurses provide skilled nursing care to patients at home. Skilled or maintenance care is provided in the home to recovering, disabled, chronically or terminally ill, people who may need medical, nursing, social or therapeutic treatments or assistance with ongoing care that cannot easily be provided solely by family or friends.
Services may include:
- Followup after hospitalization/surgery
- Health assessment
- Liaison between you and health care provider
- Management of anti-coagulation therapy
- Management of chronic conditions
- Medication set-up and monitoring including insulin
- Teaching
- Wound care
Home Health Aides
Home Health Aides are specially trained to assist in the direct care of patients at home. They are assigned and supervised by the nurse.
- Aides might visit to:
- Assist with bathing and personal care
- Assist with hair and oral care
- Assist with a home exercise program
- Encourage independence
- Prepare meals and light housekeeping
- Provide emotional support
- Provide respite care
Homemakers
Homemaker services are limited to veterans and patients eligible for one of the waiver grant programs.
Physical, Occupational and Speech Therapy
Therapy visits are available to eligible patients requiring home-bound skilled therapy services. Therapists provide treatment and exercise programs as ordered by the physician.

If not covered, patients are charged on a sliding fee scale based on their ability to pay.
