- Adult Day Care
- Adult Foster Care
- Assisted Living
- Chore Services
- Home Delivered Meals
- Home Health Aide
- Homemaker
- Personal Care
- Residential Care Services
- Respite Care
- Service Coordination
- Skilled Nursing
- Special Supplies and Equipment
Eligibility
Participant must be:
- Age 65 and over
- At risk for nursing home placement
- Financially eligible

Eligibility
Participants must be:
- At risk for nursing home placement or requires neurobehavioral level of care
- Certified as disabled
- Diagnosed with a brain injury (not congenital or degeneration)
- Eligible for Medical Assistance
- Under age 65
- With significant behavioral and cognitive problems related to the injury
